About Care Path
Care Path is a comprehensive geriataric care management service that helps elders and their loved ones cope with the challenges of aging to maximize their quality of life.
We are an affiliate of Park Slope Geriatric Day Center, a nonprofit organization that brings over 25 years of experience in meeting the needs of seniors.
